Person Centered Planning
Person Centered Planning (PCP) is about your future and reaching your goals. The PCP process leads to a written plan that is consistent with your needs and desired outcomes. The individual choices decided through PCP about what you want and need, as well as what services and supports will help you reach your goals, are used to inform your Individual Program Plan (IPP).

Pre-Enrollment Transition Support
Pre-Enrollment Transition Support includes the following:
Advocating your behalf
Participating in the IPP meeting and assisting you in negotiating an individual budget
Creating a spending plan based on an approved budget
Help finding services and resources
Onboarding with the Financial Management Service (FMS)

Self-Determination Program
Self-Determination Program participants receive one-time funding from Regional Center for PCP development and the transition into the SDP program. Participants do not pay for these services directly.
